Book your appointment

Call or e-mail us at info@828dentalavl.com and we will answer any questions you have and schedule an appointment for you. We require a confirmation of all appointments either by text message or phone call.

Please Note: There will be a $75 cancellation/no-show fee if you are unable to make your appointment without giving at least 24 hours prior notice. A cancellation made with less than a 24 hour notice significantly limits our ability to make the appointment available for another patient in need.

New patient forms

Once you book your appointment, we’ll send you some paperwork to compete. Just fill it out on your device and send it back.

Payment options

We accept Visa, MasterCard, and work with many dental insurance plans for established patients as a form of payment.

Non-established, emergency patients will be required to file insurance claims and pay for treatment at the time of the visit. Unfortunately, we do not accept Medicaid.

We will happily verify your insurance before you arrive. Please e-mail us a photo of the front and back of your insurance card along with the primary account holders birthday. Please note that e-mail is not HIPAA Compliant. If you do not wish to email your information, feel free to call us instead.
